oVigilance
nWe are the “guardians” of the patients in the O.R.
oMultiple “monitoring devices” assist us in maintaining that vigilance
nHowever, WE are the most important monitor
oPurpose: Assist with data collection regarding patient’s physiologic status & guide the administration of the anesthetic |

ASA standards for basic anesthetic monitoring: |
|
Definition
oStandard I
nQualified anesthesia personnel shall be present in the room throughout the conduct of all general anesthetics, regional anesthetics and monitored anesthesia care.
Standard II
nDuring all anesthetics, the patient’s oxygenation, ventilation, circulation and temperature shall be continually evaluated.

nInspired gas
oOxygen analyzer
oOn inspired limb of circuit
nBlood oxygenation
oPulse oximeter
oAdequate visual inspection of pt |

pTo ensure the adequacy of the patient's circulatory function during all anesthetics.
nEKG (continuous)
nBp, Hr (q 5 min.)
nIn addition: palpation of a pulse, auscultation of heart sounds, monitoring of a tracing of intra-arterial pressure, ultrasound peripheral pulse monitoring, or pulse oximetry; visualization of pt |

pTo aid in the maintenance of appropriate body temperature during all anesthetics.
nwhen clinically significant changes in body temperature are intended, anticipated or suspected. |

1)PULSE OX: ALWAYS on….. 1st on; last off……
2)BP: Check and record a minimum of every 5 min.
3)EKG:If doing a general anesthetic:
pMUST have an oxygen analyzer with a low O2 alarm limit
nThis measures the % of O2 being delivered to the pt
nLow limit is 30% (may be 25% if using heli-ox)
pIf using ventilator
nMUST have disconnect alarm, ETCO2 and stethoscope |
