Term
Definition of CP
Definition
CP describes a group of disorders of the development of mov't and posture, causing activity limitation, that are attributed to NON-PROGRESSIVE disturbances that occurred in the developing fetal or infant brain. The motor disorders of CP are often accompanied by disturbances of sensation, cognition, communication, perception, and/or behavior, and/or by a seizure disorder.
Term
Pathophysiology of CP
Definition
Neurologic insult to the immature brain at any time before birth up to postnatal per. (2 yrs); related to hypoxic ischemic encephalopaty and grade III/IV IVH
Term
Common impairments in CP
Definition
hypertonicity/spasticity, muscle stretch reflexes, primitive reflexes, dec. postural rxns, inadequate force production and control, poor selective control, inapprop. co-contraction, stereotypical mov't w/ little variety, dec. balance control, mov't has high energy cost, sensory processing impairments, abnormal involuntary mov'ts (athetoid)
Term
Associated impairments in CP
Definition
vision/hearing impairments, seizures, learning disabilities/cognitive deficits, behavioral problems, feeding and swallowing problems
Term
Incidence of CP
Definition
most common and severe MOTOR disability in childhood
Term
Etiology of CP
Definition
ischemia, hypoxia, infection, toxin, trauma
Term
Antenatal risk factors
Definition
maternal infections, intrauterine growth retardation, brain malformation, hypoxic-ischemic brain injury, multiple births, poor prenatal nutrition
Term
Perinatal risk factors
Definition
hypoxic-ischemic brain injury (anoxic trauma), premature birth, LBW, placental vascular lesions
Term
Postnatal risk factors
Definition
encephalitis and other infections, hypoxic-ischemic brain injury, IVH
Term
hemiplegia
Definition
injury in the distribution to the middle cerebral a.; periventricular atrophy
Term
quadriplegia
Definition
multiple cystic lesions in the white matter; diffuse cortical atrophy
Term
athetoid
Definition
lesion(s) in basal ganglia
Term
types of possible motor impairment in CP
Definition
spasticity, dyskinesia (dystonia and choreoathetosis), ataxia, or mixed
Term
location of -plegia
Definition
quadriplegia (6%), diplegia (44%), hemiplegia (33%)
Term
prognosis for walking
Definition
1. children w/ hemiplegia and who sit independently will most likely walk by age 2
2. poor prognosis-persistent primitive reflexes
3. all children w/ CP who will eventually walk do so by 7-8 y/o
Term
good prognosis for walking
Definition
1. head control by 9 mo.
2. sit indep. by 24 mo.
3. crawled by 30 mo.
Term
poor prognosis for walking
Definition
1. no head control by 20 mo.
2. can't sit indep. by 36 mo.
3. not crawling by 61 mo.
Term
life-expectancy
Definition
most will survive into adulthood; may have dec. life expectancy if have severe mobility and feeding difficulties; fxnal changes, including mobility changes, occur and can affect QOL issues
Term
Participation for kids w/ CP
Definition
limited opportunities to participate in age-approp. activities in the home, school, community, work force; limited education achievement; limited peer and social relations; only about 40% are gainfully employed
Term
Effective interventions
Definition
orthotics, muscle strengthening, horseback riding, constraint-induced therapy
Term
Problems in adults with CP
Definition
significant pain, poor dental health, hip/back deformities, bladder/bowel prob., fatigue, malnutrition, changes in mobility
