ICS FAILURE

Definition

Indication
=> Loss of Audio tone and/or no response to voice commands

 

CORRECTIVE ACTION

1. Perform an ICS check, check hoist pendant ICS switch and visually inspect cord. If in position to safely do so, retrieve the device(FM).
2. Perform an ICS Check, check PTT and cord (P).
3. Slowly move the aircraft to establish visual reference with the hoisting area(P).
4. Payout slack - AS REQ'D(FM)
5. Troubleshoot ICS Malfunction.

If ICS malfunction is corrected, proceed with normal hoist procedures.

If ICS malfunction is not corrected:
6. Position aircraft to recover rescue device per Rescue Briefing
7. Signal pilot to reestablish visual reference with hoisting area by pointing to the left side of aircraft. Rescue device may or may not have been recovered(FM).
8. Observe signal, pass to pilot(CP)
9. Slowly move aircraft to establish visual reference with the hoisting area(P).

If device was recovered:
10. Complete hoist of rescue device (FM).

If the device was not recovered:
11. Make onscene repairs, abort the mission, or take other action as appropriate

EMERGENCY RECOVERY OF THE RESCUE SWIMMER

Definition

If aircrew on the helicopter observe the IN TROUBLE NEED ASSISTANCE hand signal or see a potential hazard to the swimmer such as dangerous marine life, the helicopter shall move into position for an emergency pickup of the swimmer using one of the following devices:

1. Rescue Basket - if rigged and empty. The rescue basket is the primary device for recovery

2. Bare Hoist Hook - rescue basket is not readily available.

If the swimmer or device is attached to the hoist hook between the water and beneath cabin door:

EMERGENCY EGRESS PROCEDURE

Definition

1. Reference Point - Locate and remain calm

VORTEX RING STATE

Definition

Indications:

Aircraft in a vertical descent with max power applied and little or no cyclic authority. Nr remains at 100%. May be entered any time the Main rotor is in a state of high upflow and low longitudinal airspeed such as quick stop type manuevers or during autorotative recoveries. Power should be increase once airspeed is above approx 20KIAS

CORRECTIVE ACTION:

1. Decrease collective pitch

2. Increase forward airspeed or establish directional flight

3. Enter autorotation if altitude permits

BLADE STALL

Definition

CORRECTIVE ACTION:

1. Decrease collective pitch

2. Decrease severity of manuever

3. Gradually decrease airspeed

4. Increase rotor RPM

5. Decrease altitude

Brief Discussion:

The retreating blad will stall since it is traveling at the rotational velocity, minus the forward speed of the helicopter.

Most likely to occur when operating at high values of speed gross weight, density altitude, power and aggrevated by low Nr.

Manuevers, acceleration, or turbulent air, all of which increase g-load factors, reduce the airspeed at which blade stall will occur.

In a blade stall, each main rotor blade will stall as it passes through the stall region and create vibrations per revolution equal to the number of blades.

Fully developed blade stall will induce a pitch up and left. Forward cyclic is ineffective at this point and may aggrevate the condition because it increase the angle of attack of the retreating blade.

6) Land as soon as practicable

CORRECTIVE ACTION:

1) LAND IMMEDIATELY - Use minimum power required to fly at a safe altitude and airspeed for entry into autorotation if tail rotor drive failure occurs.

Definition

Symptoms:

- TQ, Ng, Np, and TGT of both engines, if indicated, oscillate out-of-phase.
- Nr oscillates in phase with malfunctioning engine.

Symptoms:

- Nr at 100% in low to normal power situations and decays below 100% as high power is demanded

- Ng, TGT, and/or torque of the malfunctioning engine may be erroneous or absent

Definition

CORRECTIVE ACTION:

1) Engine Malfunction Procedure

- Rapid increase in TGT
