Term
So you've treated a patient with I-131. What must you do before you can discharge the patient?
Definition
  1. Confine pt until dose to a member of the public will not exceed 0.5 rem (5 mSv).
  2. You can calculate dose estimation based on tissue shielding and biological clearance.
  3. Must retain documentation for 3 years.
Term
You've given a nursing mother Tc-99m MDP (bonescan) or Tc99m MIBI (cardiac). How long must they interrupt breastfeeding?
Definition

3 hours.

Duke 2009

Term
A nursing mother gets Tc99m-MAA for a VQ scan. How long should they interrupt breastfeeding?
Definition

3 to 12 hours.

Duke 2009

Term
A nursing mother gets a thallium myocardial stress test.  How long must they stop breastfeeding for?
Definition

48-96 hours

Duke 2009

Term
A pt gets I-131 for a thyroid scan. How long before they can resume breastfeeding?
Definition

Must cease nursing.

For I-131 > 0.4 microCuries.

Duke 2009

Term
What is the general regulatory requirement for radiopharmaceutical administration and nursing women?
Definition

Interrupt nursing if the dose to the infant could exceed 100 mrem.

10 CFR 35.75

Term

You give oral Na131I to a patient and find out 3 hours later she is 5 weeks pregnant. You should immediately (CHOOSE ONE):

  1. Give SSKI to block fetal thyroid
  2. IV fluids, encourage frequent voiding
  3. Induce vomiting, then send to ED for gastric lavage.
Definition

Answer: 2. IV fluids, encourage frequent voiding. Will reduce gamma dose to infant.

Fetal thyroid doesn't take up iodine before 10-12 weeks.

Already absorbed after 1-2 hours.

Term

True or False, which of these is a reportable "medical event":

  1. Wrong patient
  2. Wrong radiopharmaceutical
  3. Wrong route of administration
  4. Administered activity > 20% of prescribed
  5. Unintended administration to fetus if fetal dose > 10 rem.
Definition

Answer:

#1-4 TRUE only if also: EDE > 5 rem or organ dose > 50 rem

#5 FALSE: Report if fetal dose > 5 rem.

Term
What's the criteria for Minor and Major Spill of Tc-99m and Thallium-201?
Definition

Both:

Minor < 100 mCi

Major > 100 mCi

Term
What's the criteria for Minor and Major Spill of Ga-67, In-111, I-123, and Sm-153?
Definition

Minor < 10 mCi

Major > 10 mCi

Term
What's the criteria for Minor and Major Spill of I-131, P-32, and Sr-89?
Definition

Minor < 1 mCi

Major > 1 mCi

Term
You spill 30 mCi of Tc-99m MIBI on the floor. What do you do?
Definition

This is a Minor Spill. So you would

  1. Notify persons in the area about the spill.
  2. Cover spill with absorbent paper to contain it.
  3. Put on gloves, lab coat, and booties. Clean up with the paper, carefully folding it and place in a labeled bag and everything into the radioactive waste container.
  4. Survey the area with a counter, check hands, clothing, shoes for contamination.
  5. Report to Radiation Safety Officer.
Term
You spill 5 mCi of NaI-131 on the floor. What do you do?
Definition

This is a Major Spill. You would:

  1. Vacate everybody from the room.
  2. Cover spill with absorbent paper labeled "Caution Radioactive Material". Fence off the area, and isolate those people who may be contaminated.
  3. Notify Radiation Safety Officer immediately.
  4. Decontaminate personnel by removing clothing, flushing skin with lukewarm water and mild soap.
  5. DO NOT TRY TO CLEAN IT UP YOURSELF.
Term
What is the DOT White Triangle I Package Label supposed to indicate?
Definition

Exposure of < 0.5 mR/hr on contact.

Transport Index negligible at 1 meter.

Term
What is the DOT Yellow II Package Label supposed to indicate?
Definition

Exposure < 50 mR/hr on contact

Transport Index < 1 mR/hr at 1 meter

Term
What is the DOT Yellow Triangle III Package Label supposed to indicate?
Definition

Exposure < 200 mR/hr on contact

Transport Index < 10 mR/hr at 1 meter

Term
What are the steps to open a package containing radioactivity?
Definition
  1. Put on gloves.
  2. Visually inspect for damage, if so notify RSO.
  3. Check external surfaces with survey meter.
  4. Open outer and inner packages, verifying with packing slip.
  5. Check source container for damage or leaks, if so notify RSO.
  6. Do a wipe test if contamination is suspected.
  7. Monitor empty packaging for contamination with survey meter before discarding.
  8. Either treat as radioactive waste, if not then obliterate labels before discarding.
  9. Make a record of the receipt.
Term
What are the three key points to remember when returning a used generator back to the manufacturer?
Definition

Save the original box (DOT-approved container).

Wipe test everything.

Retain records of receipts and transfers for 3 years.

Term
How often should you check Constancy?
Definition
Daily
Term
How often should you check Linearity on a dose calibrator?
Definition
Upon installation or after repair, then QUARTERLY.
Term
How often should you check Accuracy?
Definition
Upon installation or after repair, then ANNUALLY.
