Term
what are characteristics of BED?
Definition
Recurrent episodes of binge eating, an episode being characterized by both of the following:

(a) Eating, in a discrete amount of time (e.g. within a 2-hour time period), an amount that is definitely larger than most people would eat during a similar period of time in similar circumstances

(b) A sense of lack of control during the episodes, for example, a feeling that one can’t stop eating or control what or how much one is eating

3. Marked distress about binge eating

4. Frequency of 2 days per week for 6 months

5. Does not occur only during the course of bulimia nervosa or anorexia nervosa
Term
what is the treatment of choice for BED?
Definition
There had been the belief that weight loss required the control of binge eating.
In fact, it was believed that attempted weight loss of binge eaters would precipitate more binge eating
However, behavioral weight control programs that pay no attention to binge eating not only reduce weight of binge eaters but also their rate of binge eating.
This finding suggests that behavioral weight control is the treatment of choice for obese binge eaters
Term
what are comorbid diagnoses with BED?
Definition
Major Depression, Substance Use, Anxiety Disorder, Axis I Disorder Personality Disorder
Term
what is BED a marker of?
Definition
The very high comorbidity of Binge Eating Disorder suggests that it may most usefully serve as a marker for specific disorders such as depression, anxiety and substance abuse.
Treatment specific to these disorders may have a far greater impact than treatment directed at Binge Eating Disorder.
Term
what are 2 salient points about NES?
Definition
Contributes to Obesity and it

Can be controlled
Term
what are the provisional criteria of NES?
Definition
Morning anorexia, even if the subject eats breakfast;
Evening hyperphagia, >35% of kcals consumed after last meal;
Awakenings at least once per night;
Consumption of snacks during awakenings;
Repetition of pattern for at least 3 months;
Does not meet criteria for another eating disorder
Term
what is common program of NES pts?
Definition
wake up (1 min) eat (4-5minutes), go back to bed in 20 minutes. eating gets you back to sleep. this becomes a conditioned reflex.
Term
What is NES's relationship with Circadian rhythms?
Definition
The Night Eating Syndrome represents a dysregulation of circadian patterns of food intake without disruption of the circadian sleep pattern
NES is primarily an eating disorder and only secondarily a sleep disorder
The first clinical example of the dissociation of circadian rhythms that is attracting great attention in biology
Term
Do SSRIs, like setraline, work?
Definition
In an RTC, 34 severe night eaters received either sertraline or placebo for 8 weeks
There was a dramatic decrease in all aspects of the Night Eating Syndrome
Term
what is the summary?
Definition
NES is characterized by morning anorexia, evening hyperphagia, and sleep disturbance with awakenings followed by nocturnal ingestions.
Depressed mood is common.
It occurs in both normal weight and obese persons.
The food intake of night eaters reflects a delay in its circadian rhythm while the circadian sleep rhythm is intact.
A family study of night eaters shows an Odds Ratio of 4.9 compared to control subjects.
Treatment with the SSRI sertaline significantly reduced hyperphagia, awakenings, and night ingestions
SPECT studies show a marked elevation in the serotonin transporter among night eaters compared to depressed patients and control subjects
