Uses:Major depressive disorder,generalized anxiety disorder

Action:inhibits CNS neuonal reuptake of serotonin

Side effects:nausea,dry mouth,increased sweating,rhinitis

Nursing cons.:do not take with Celexa,d/c lexapro may cause dysphoric mood,irritablity,agitation,anxiety,hypomania,insomnia-gradually decrease dose

Dose:oral solution and tablets

depressive disorder-10 mg/day, may increase to 20mg/day

GAD-10 mg/day, may increase to 20 mg/day

Lorazepam

Class:Antianxiety,Benzodiazepine

Action:redfuces anxiety by increasing or facilitating the inhibitory neurotransmitter activity of GABA

side effects:drowsiness,fall risk,ataxia,confusion

Nursing cons.:increase evening doses before daytime doses;may be necessary to increase dose in females taking oral contraceptives;with long term txgradually decrease dose to d/c

Dose:oral concentrate,tablets,IM,IV

Anxiety-2-3mg/day,can range from 1-10mg/day in divided doses-2-4mg at bedtime

IM-0.05mg/kg max of 4mg

IV-4mg slow push for status epilepticus

Preanesthetic-2mg or 0.044mg/kg whichever is the lesser dose

Purpose for my pt.-anxiety

Dose: 0.5mg by mouth once in the evening

Class:Diuretic,Thiazide

Uses:once daily low dose treatment of HTN

Action:promote excretion of sodium and chloride, thus water, by the distal renal tubule. Increased excreationof Potassium.Direct dialtion of arterioles and reduction in total fliud volume of body

Side effects:orthostatic hypotension,hypoalemia,weakness

Nursing cons.:change positions slowly(dizziness),take in the morning to avoid nightime urinary frequency

Dose:capsules,oral solution,tablets

diuretic-25-200mg/day until dry weight achieved

antihypertensive-25mg/day single dose, may be increased to 50mg/day

Class:Thyroid product

Uses:replacement or supplemental therapy for hypothyroid,treatment or prevention of nodules or goiters,adjunct to surgery and radioactive iodine therapy

Action:synthetic sodium salt of the levoisomer of T4

Side effects:palpations,nervousness,insomnia,weight loss

Nursing cons.:Take on empty stomach or 2-3 hours after meal,avoid direct or artifiacial sunlight,alternate method of birth control-may interfere with oral contraceptives

Dose:tablets

mild hypothryoidism-50mcg/day, increase to desired dose (75-125mcg/day

severe-12.5-25mcg/day, increase to desired dose

Zocor

Class:Antihyperlipidemic

Uses:adjunct to diet to reduce elevated total and LDL cholesterol and Triglyceride levels,increases HDL levels

Action:hydrolized to simivistatin acid, inhibits enzyme that is the early atep in the biosynthetic pathway for cholesterol

Side effects:abdominal cramps,URTI,flatulence

Nursing consid.:low cholesterol diet must be followed during drug therapy;report muscle weakness,pain/tenderness,regulare exercise

Dose:tablets

hyperlipidemia,coronary heart disease-20mg once daily in evening, may increase to 5-80mg/day

Purpose for my pt.-hypercholesterolemia

Dose-40mg by mouth once daily

Propranolol Hydrochloride

Class:Beta-Adrenergic blocking agent

Uses:HTN,angina pectoris,MI,essential tremor,migraine,cardiac arrhythmia

Action:decreases cardiac output,inhibition of renin;for angina it reduces oxygen requirement of heart

Side effects:insomnia,anxiety,impotence,nervousness,lightheadedness

Nursing consid.:may mask signs of low blood sugar,may cause increased sensitivity to cold,don not administer for 2 weeks after use of MAO drug use

Dose:capsules(extendedrelease,oralsolution,oralsolution oncentrated,tablets

HTN-40mg bid or 80mg XR once daily, may increase to maintain

Angina-80-320mg 2-4 times per day or 80mg XR once daily

Arrythmias-10-30mg 3-4 times daily

MI-180-240mg/day in 3-4 divided doses

Nursing consid.:do not exceed prescribed dose,avoid alocohol,may cause dependence,assess for depression and suicidal ideations

mild-mod. pain-100-650mg every 4 hours not to exceed 600mg of proxyphene daily

Dose-100/650mg tab by mouth once every 6 hours prn for moderate neck pain

        100/650mg tab by mouth two tabs every 6 hours prn for severe neck pain

Class:antidiarrheal

Uses:symptomatic relief of acute nonspecific diarrhea,chronic diarrhea associated with inflammatory bowel disease,decrease volume of discharge from ileostomies

Action:slows intestinal motility,prolonged retention of feces results in reducing volume of stools, increase in viscosity

Side effects:abdomianl pain/distention/discomfort,dry mouth

Nursing consid.:recor dnumber of stool sper day,d/c after 48 hours if ineffective,may cause dizziness

Dose:capsules,OTC capsules,liquid,tablets

acute diarrhea-4mg, followed by 2 mg after each unformed stool, up to 16mg/day

chronic diarrhea-4-8mg/day

Side effects:drowsiness,constipation,dry mouth/nose/throat

Nursing consid.:photsensitvity reaction,take 30 minutes prior to travel for motion sickness,avoid alcohol

Dose:capsules,capsules soft-gel,elixir,liquid,oral solution, syrup,tablets,tablets

hypersensivity,motion sickness,parkinsonism-25-50mg po 3-4 times daily , not to exceed 300mg/day

sleep aid-50mg at hour of sleep

Uses:prphylaxis and treatment of iron deficient anemias,dietary supplement for iron

Action:Iron absorped from duodenum and upper jejunum by an active mechanism through mucosal cells where it combines with the protein transferrin

Side effects:constipation,anorexia,dark colored stools

Nursing consid.:take with meals,increase fiber to minimize constipation,may cause indigestion

Dose:drops,elixir,tablets

prophylaxis of anemia-300mg/day

anemia-300mg bid,may increase to 300mg qid prn

oxybutynin chloride

Class:antispasmodic,anticholinergic agent

Action:direct antispasmodic effect on smooth muscle and inhibits muscarinic action of acetycholine on smooth muscle

Side effects:drowsiness,blurred vision 

Nursing consid.:take pill same time every day,do not crush or chew,be aware of high temp. due to decreased ability to sweat

Dose:tablets(extended release)

5-10mg/day, may adjust in 5mg intervals

Purpose for my pt.:urinary incontinence

Dose:5mg by mouth once daily

Cranberry tablets

Class:herbal

Uses:prevention and treatment of urinary tract infections,urinary deodorizer for incontinent persons.

Dose:3oz. juice daily for preventing UTIs and 12-32oz. daily to treat UTIs.

Capsules-6/day of dried cranberry powder or 300-400mg concentrated juice caplets 2 tiimes per day

Nusring consid.:avoid using amounts greater than consumed in food during pregnancy and lactation,may increase absorption of vitamin B12 in clients taking PPIs

Side effects:no side effects,consuming more than 3-4L/day can cause diarrhea and other GI symptoms
