Term
What size needle would you use for an IM injection?
Definition

IM: 20-25 gauge

1-3 in (depending on the size of patient and iscosity of the drug)

Term
Where would you give an IM?
Definition

IM should be given:

deltoid

dorsogluteal

vetrogluteal

vastus lateralis

Term
[image]
Definition
  • Dorsogluteal
  • Draw an imaginary line from the posterior superior iliac spine to the greater trochanter.
  • Insert the needle at a 90-degree angle above and outside the drawn line.
  • You can administer a Z-track injection through this site. After drawing up the drug, change the needle, displace the skin lateral to the injection site, withdraw the needle, and then release the skin.

(always aspirate first)

Term
[image]
Definition
  • Deltoid
  • Locate the lower edge of the acromial process.
  • Insert the needle 1" to 2" below the acromial process at a 90-degree angle
  • Always aspirate
Term

Definition

Vastus lateralis and rectus femoris sites

  • Find the lateral quadriceps muscle for the vastus lateralis, or the anterior thigh for the rectus femoris.
  • Insert the needle at a 90-degree angle into the middle third of the muscle, parallel to the skin surface.
  • Always aspirate
Term
[image]
Definition

Ventrogluteal site

  • With the palm of your hand, locate the greater trochanter of the femur.
  • SSpread your index and middle fingers posteriorly from the anterior superior iliac spine to the furthest area possible. This is the correct injection site.
  • Remove your fingers and insert the needle at a 90-degree angle.
  • Always aspirate
Term

Definition

Sites for SQ injections

  • Pinch (elevate) SQ tissue 1 in.
  • Holding the syringe the way you would a pencil or dart, insert the needle at about a 45 degree angle to the pinched-up skin (the needle should be completely covered by skin).  (90 degrees for heparin)
  • Aspirate (unless giving heparin)
  • inject
  • massage site gently (unless contraindidcated as with heparin)
  • No more than 2 mL
  • adult 25 gauge 5/8 inch
  • infant, child, elderly 25- 27 gauge 1/2 in.
Term
What position do you put the patient into for a rectal supository?
Definition
Patient should be on his Left side in a Sims position, drape appropriately.
Term
How much medication can you give by IM?
Definition
IM: Up to 5 mL can be given.  If over 3 mL is given- consider administering 2 separate injections
Term
What angle do you inject an IM at?
Definition
IM- 90 degrees  (always aspirate first)
Term
What is z-track?
Definition
Z-track: method of IM injection to prevent leaking (tracking) of medication into the SC tissue.  Use z-track when administering medications that will irritate and discolor the tissue (i.e. iron)
Term
When should you use z-track?
Definition

Use z-track when administering medications that will irritate and discolor the tissue (i.e. iron)

This IM method prevents leaking/ tracking of medication into the SC tissue.

Use 2 20 gauge needles- at least 2 in.

Use large muscle mass- gluteal

Term
What steps do you use when preparing medication for injection by z-track?
Definition
  • Use 2 20 gauge needles  (at least 2 inches)
  • draw up medication with 0.2- 0.5 mL of air
  • remove first neelde and replace with unused second needle  (prevents irritation to sc tissue as needle is inserted.
  • choose a large muscle mass - gluteal
  • displace the skin laterally by pulling it 1 in. away from injection site.
  • Inject deeply at a 90 degree angle
  • ASPIRATE
  • Inject slowly
  • wait 10 seconds before withdrawing needle
  • Withdraw (slowly???)
  • Release displaced skin and sc tissue
  • do not massage
  • encourage ROM or walk to facilitate absorption
  • Alternate sites
