it is the placement of plates in the face to fixate the jaw, different plates can be used of different shapes and thickness. Sometimes the entire jaw is wired shut to help healing. The plates used are called arch bars. They are not commonly used because fixation techniques have improved and bettered the recovery of patients. |

What kind of equipment will be used?

Definition
Outside of the normal equiptment, a bovie and ortho set are needed. Also needed are arch bars with varying diamater as well as screws and steel wire, again of varying sizes. If arch bars are to be applied, an arch bar set should be available. |

Stretch them... To ensure that the wires do not stretch during or after surgery, you must place them in the wire jaws and stretch them just a bit and cut off the crimped ends. This must be done for each wire and several will be used.. always tighten and loosen in the correct direction.. righty tighty, lefty loosy... |

How is the mouth protected during surgery? |

Definition
To protect the mouth, use a mouth prop and plastic cheek retractor. Also throat packs can be inserted. They must be removed before the surgery is over because THE MOUTH WILL BE WIRED SHUT. Carefully complete counts before the jaw is wried shut! |

Definition
After wires are prepared, pass with caution, the ends can easily puncture. Place end in wire jaws and make sure they are secure.

Definition
Suction and water/NS to rinse out the mouth. After the jaw is wired shut, it should be cleaned. After retractors are removed depnding on the case, sutures may or may not be needed. |

During recovery, why are the wire cutters sent with the patient? |

Definition
Vomiting or respiratory distress may occur in which case the mouth will need to be opened to alleviate symptoms. A tracheotomy may even be nessesary. |

4-6 weeks if no complication occurs. Complications include but are not limited to hemorrage, postoperative SSI, misalignment/malocclusion. |

To ensure that the fracture has been reduced and also that the screws are not protuding too far but are getting a good bite into the bone. |

Screw fixation is when screws are placed through a plate to fixate the mandible for healing. Arch bars are used on the teeth, wrapping around teeth to fixate the mouth shut so the bones are compressed for healing. Sometimes both methods are used in severe fractures. |

No prep is needed because it is a clean contaminated case AKA, class 2 of the mouth area. However, sometimes teeth may be brushed and a mild antiseptic wash may be used. |
