Term
Name the 4 pressure tolerant areas along the lateral surface for a transtibial amputation.
Definition
Patellar Tendon
Medial Tibial Flare
Along both sides of the tibia
Medial and lateral femoral condyles
Term
Name 2 pressure tolerant areas along the lateral surface for a transtibial amputation
Definition
Anterolateral compartment
Shaft of the fibula
Term
Name the pressure tolerant area along the posterior surface for a transtibial amputation.
Definition
Most structures within the popliteal fossa
Term
__ areas can take pressue, but __ areas cannot.
Definition
Fleshy
Bony
Term
Name 4 aspects of evaluating an amputation to see if limb is fitting into socket properly.
Definition
Visual (skin check)
Powder Test
Ball of Clay Test
Lipstick Test
Term
What is the powder test?
Definition
Tells you where the pt fits in the socket. Put powder on their limb and see where in the socket also has powder on it after pt puts limb in.
Term
What is the ball of clay test?
Definition
You put a ball of clay into the bottom of the socket then stick limb in there to see how much of the clay is smushed. If it's still round then limb isn't going deep enough, if it is really flat, then limb is too far into socket.
Term
Name 10 possible common skin disorders than may be associated with amputations.
Definition
Maceration
Dermatitis
Blister
Abrasion and Erosion
Ulceration
Bacterial Infections
- Folliculitis
- Furnucle
- Carbuncle
- Cellulitis
- Osteomyelitis
Edema
Epidermal Cyst
Eczema
Verrucous Hyperplasis (BAD!)
Term
May be caused by an allergic reaction to lotion or a gel liner. May occur from too much soap on the distal end or lotion.
Definition
Contact Dermatitis
Term
May be caused by the use of cream, lubricant or cleansing agent as well as padding inside the socket. This could occur with new usage or after prolonged use. May be caused by the coating used inside the socket.
Definition
Contact Dermatitis
Term
Name 2 treatments for Contact Dermatitis
Definition
Identification and removal of irritant
Cool compresses and hydrocortisone
Term
How might a blister result in an increased risk of infection
Definition
if it busts open
Term
May be caused by prosthesis rubbing against skin, will require several weeks of healing without prosthesis.
Definition
Blister
Term
While this is not necessarily an open wound, prosthetic wear should not be done until healed. May be secondary to mechanical rub.
Definition
Erosion
Term
This type of skin disorder is normal if mild. You can wear the socket with this, but monitor closely. May be secondary to mechanical rubbing.
Definition
Thickened skin (callous)
Term
Can you wear your socket with scars and ulcerations?
Definition
Scars yes, but its not great for WBing
Ulcerations, NO
Term
If you have an abscess from a bacterial infection, can you continue to wear your socket?
Definition
NO
Term
Often seen with individuals with hairy, oily skin and aggravated by sweating and rub from the socket wall. Worse in hot months, not serious for most however in pts with diabetes it can lead to difficulties. Good hygiene is the most effective treatment.
Definition
Furuncle/Boil
Term
This may be caused by the residual limb not going into socket deep enough.
Definition
Edema
Hemorrhage
Term
May be due to hair follicle issues especially above the knee or on the medial side
Definition
Epidermal Cyst
Term
Typically does not appear until the prosthesis has been worn a few months or years and occurs most often in transfemoral amputations in the upper medial margin of the residual limb
Definition
Epidermal Cyst
Term
How can epidermal cysts be treated?
Definition
-Experimental, if the cyst continues to enlarge surgery may be indicated
-Topical steroids
-Socket lines - trial
Term
Most often caused by improper fit of socket, with improved fit the problem resolves, use of topical steroids may also help. Can keep wearing socket but proper fit must be assessed.
Definition
Eczema
Term
The most common wart virus of the skin. The distal end of the limb does not have adequate support and dangles in the socket.
Definition
Verrucose Hyperplasia
Term
How can varrucose Hyperplasia be treated?
Definition
External compression in combination with adequate control of bacterial infection and edema. Total contact in socket. Shrinker when not in socket
Term
How do you know early on that pt is changing size of their limb?
Definition
Girth measurements. Have proper landmarks and document well. Do this initially and periodically
Term
When do you evaluate prosthesis?
Definition
Everytime when donning and doffing
Term
3 ways to evaluate the cause of redness or discomfort
Definition
Pill or powder test
Eliminate cause
Know when to consult prosthetist
