Term
Congenital Malformation of Ossicles
Definition
-deformity during fetal devel.
-fusion of some/all structures
-can have max Conductive loss (60dB)
-Type As tympanogram
-Good results w/ prosthesis +/or h. aids
Term
Ossicular Disarticulation
Definition
-Cause: trauma / cholestiatoma
-Incudostapedial (joint btwn stapes & incus)
-Minimum loss +/- 30-40dB, max 60dB
-Management: surgical repair (put sleeve on joint), prosthesis
-Tympanogram: floppy, Ad type.
-Note: may get SN at 4k b/c acoustic trama of hitting head).
Term
Incudostapedial
Definition
Joint betwix the incus and stapes
Term
Disorders of Tympanum
Definition
Cholestiatoma; Otosclerosis; E. Tube Dysfunc; Otitis Media;
Term
Cholestiatoma
Definition
-Middle ear cyst related to chronic O. Media.
-Pars flaccida suckd in by -pressure, made of skin, sloughs off, ad infinitum
-HL: mild - severe (depends on locale)
-management: surgical (can impact chorda tympani, ossicles, etc)
-NOTE: low frequencies can be preserved,
-Tympanogram: variable b/c cyst can be all over the place
Term
Otosclerosis
Definition
-ME ossicles overgrown w/ bone (usu. at footplate of stapes)
-Stiffens ossicular chain
-Common in women during child b. year, can be related to pregnancy, etiology unknown (prob genetic)
-HL: progressive, notch at 2k (Carhardt's notch) due to resonant freqs.
-Tympanogram: Type As
-Manage: surgery, stapedectomy, stapeotomy
Term
Otosclerosis
Definition
-ME ossicles overgrown w/ bone (usu. at footplate of stapes)
-Stiffens ossicular chain

-HL: progressive, notch at 2k (Carhardt's notch) due to resonant freqs.
-Tympanogram: Type As
-Manage: surgery, stapedectomy, stapeotomy
Term
Eustachian Tube Dysfunction
Definition
-Cause: infection of nasopharynx (upper respiratory, allergies, inflammation, swelling --> air doesn't turn over, get neg. pressure in ME.
- Uni- or bi- lateral; mild loss.
-Tympanogram type c.
-Management: follow and retest, decongestant... could be getting better.
-CONCERN: kid with SN loss, this could really impact life, may treat more aggressively.
-
Term
Otitis Media
Definition
-Most common HL in children (80% have at least one bout); most prev. childhood disease; most common reason for dr. visit; myringotomy was most common surgery; billion $ anti-bio biz.
-Predisposing: males; race (+ alaskan natives, - asians); season (share germs); climate; age (12 - 24 mnts); environ (2nd hand smoke?); genetics; day care (share germs); bottle fed (maybe milk allergies)
-Dianosis: otoscopy, type b tympanogram; audiometry
-Sequence: u. resp. infection --> E tubes inflame --> can't vent --> air depletes --> neg. pressure --> mucous secreted --> infection invades ME --> infection w/ infusion
Manage: decongestants, anti-bio, myringotomy, lance TM, Pressure Equilization Tubes (PE) (grommeted tubes in TM)
Term
Complications from O Media
Definition
spontaneous tear in TM; cholestiatoma; TM Scarring; glue ear; mastoiditis; facial paralysis (chorda tympani)
Term
Congenital
Definition
Present at time of birth (not necessarily genetic)
Term
Perinatal
Definition
right around the time of birth
Term
Aquired
Definition
adult oriented, understood to be aquired in adulthood or late childhood.
Term
Mondini Dysplasia
Definition
Type of cochlear malformation; 1 turns of cochlea instead of 2.5
