FACTS ABOUT GREATER SAPHENOUS VEIN |
|
Definition
THE GSV HAS 10-13 VALVES. IT COURSES SUPERFICIALLY IN THE ANTEROMEDIAL ASPECT OF THE LEG. ITS DISTAL END IS ANTERIOR TO THE MEDIAL MALLEOLUS AND ITS PROXIMAL END IS AT THE CFV IN THE GROIN AREA. IT IS THE LONGEST VEIN IN THE BODY. ABOUT 35% OF INDIVIDUALS HAVE VARIANTS IN THE THIGH. MOST OFTEN THIS IS A BIFID SYSTEM. THE EXTERNAL PUDENDAL, SUPERFICIAL CIRCUMFLEX ILIAC VEIN AND THE INFERIOR EPIGASTRIC VEINS DRAIN INTO THE GSV AT THE SFJ. A POSTERIOR MEDIAL BRANCH OF THE GSV COMMUNICATES WITH THE GIACOMINI VEIN IN THE THIGH. THE GIACOMINI VEIN COMMUNICATES WITH THE LSV ABOVE THE SAPHENO-POPLITEAL JUNCTION. |

PERFORATORES SHOULD BE EVALUATED IN WHAT POSITION |
|
Definition
PERFORATORS SHOULD BE EVALUATED WHEN THE PATIENT IS SITTING WITH THE LEG IN A DEPENDENT POSITION OR WITH THE PATIENT STANDING. IF THE PATIENT IS STANDING, THE NON WEIGHT BEARING LEG SHOULD BE EVALUATED. PERFORATORS THAT ARE INCOMPETENT ARE USUALLY DILATED (GREATER THAN 3 MM). THESE MAY APPEAR AS HOLES IN THE FASCIA. |

HOW DO WE EVALUATE PERFORATORS |
|
Definition
A PERFORATING VEIN COURSES DEEP AND CONNECTS TO THE DEEP SYSTEM. FLOW IN COMPETENT PERFORATORS IS FROM SUPERFICIAL TO DEEP. IF FLOW IS FROM DEEP TO SUPERFICIAL, THE PERFORATOR IS INCOMPETENT. PERFORATORS TEND TO BE TORTUOUS. WHEN A PERFORATOR IS INCOMPETENT, A SKIN DEPRESSION CAN OFTEN BE PALPATED OVER IT. |

WHY RADIAL ARTERY IS CHOSEN OVER GSV FOR GRAFT |
|
Definition
FIRST OF ALL, THE RADIAL ARTERY IS OF THE APPROPRIATE CALIBER FOR THE TYPE OF BYPASS GRAFT. THE WALL OF THE ARTERY IS THICKER AND LESS PRONE TO INTIMAL HYPERPLASIA. AND LASTLY, THE ARTERY IS MORE READILY AVAILABLE FOR HARVEST. |
