 In order to change a medical record the incorrect information must be lined out and initialed 

Releasing medical records to the owner 

 Make a copy of the record

Releasing records to a third party

Authorization form must be signed by owner and a copy can be given

Exception: reportable diseases, or rabies vaccination status 

Requirements for cetification

 1. Completion of an AVMA approved program

2. Satifactory performance on the exam

3. Sixteen hours of continuing education every 2 years
